Survey Blocks is currently in public beta. Functionality will evolve, and features may be modified or removed based on user feedback and stability. 

The Survey Blocks feature is designed to help you collect actionable, anonymous feedback. Instead of assessing learning like traditional quizzes or knowledge checks, survey blocks let you gather learner insights and opinions in a flexible, user-friendly way. Select one type from the several available options, deploy your training, and then monitor responses from within your content. Here's how it works.

Step 1: Add Survey Blocks

Follow these steps to add a survey block from the block library: 

1. Open a course or microlearning.
2. Open the block library and select Survey.
3. Choose Survey. 

Step 2: Select the Survey Type

Survey blocks have multiple possible response types. Choose from:

• Linear scale: Rate using numbers on a 2 to 10-point scale 
• Sentiment: Rate positive-to-negative on a 2 to 5-option scale
• Multiple response: Select one or more options from many
• Multiple choice: Select one option from many
• Short response: Respond in an open-text field that allows up to 300 characters 
• Long response: Respond in an open-text field that allows up to 5,000 characters 

No matter which survey type you choose, the same steps apply to use it:

1. Hover over the block and click the Content icon.
2. Add an optional title to the block. This will help you find it in the engagement report.
3. Select a new survey type or modify the question. Modify the scale and sentiment labels as needed.
4. Toggle Required as needed. If Required is toggled on, learners must respond to the survey question to complete the training.
5. Click Add Item to add additional questions to the current survey. 

Survey blocks can contain as many responses as you'd like in whatever combination you like. You can also change the response type at any time. 

Pro Tip: Survey response reporting combines responses for all questions in a single survey block. If you have multiple responses that you'd like to track individually, we recommend using multiple survey blocks.

Step 3: Modify the Block Settings

Modify how your content looks on the screen by hovering over an existing block to access the left-hand design toolbar. Click the Style icon to access block background options. The Format menu lets you change the block padding.

Step 4: View Feedback

Survey block responses are available when you publish your content via Quick Share or LMS. For LMS exports, you must have the Connected Packages feature in Labs enabled to view survey responses in Rise 360. Contact your third-party LMS provider to access Survey Block data when publishing to LMS using the standard package.

To see survey block responses, go to the Publish menu and click either View Quick Share engagement or View LMS engagement, then select the Survey responses tab. Select a survey block title from the drop-down menu to see the aggregated responses. Once a survey question has at least five responses, you can generate an AI summary of all responses to date. 

Note: Generated summaries aren't persistent and must be regenerated whenever the response tab is reloaded.

When a survey question has been changed to a different response type, previous responses are preserved but aren't visible. To see previously entered responses, change the response type back to one of the text-entry response types or vice versa.  

Anonymous vs. Logged Learner Responses

The Guestbook must be enabled to include the respondent's name in short and long response answers. All other survey question types have anonymous responses. 

Note: When using LMS Export, all survey responses may have user ID data associated with them, regardless of the question type. This is due to the nature of third-party LMS providers and how they identify users.
